Astoria Medical Centre collects, uses and discloses personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). We keep your information strictly confidential.

What we collect through this website

Pre-registration: your name, date of birth, contact details, preferred clinic and physician, health coverage type, and anything you choose to tell us.

Appointment requests: your contact details, date of birth, health coverage type, the appointment you request and, optionally, the reason for your visit.

Contact messages, opening-update sign-ups and job applications: the details you enter in those forms.

Please do not send detailed health information through the contact form. Call the clinic instead.

How we use it

We use your information only for the purpose you gave it: to process your registration, respond to your appointment request or message, send the updates you asked for, or consider your application.

We do not sell your information and do not share it with third parties except as required by law or with your express consent.

How we protect it

Information is sent over an encrypted connection and stored on access-controlled systems used only by authorised clinic staff. Notification emails sent to our team do not contain your health details.

Your choices

You may withdraw your consent, unsubscribe from updates, or request access to or correction of your information at any time by contacting our office.
