What to bring to your appointment
A short checklist so your visit goes smoothly — and what to do if your health card has expired.

Please bring your health card, government-issued photo ID, an up-to-date medication list, and any relevant medical records or specialist letters. For appointments requiring preparation, follow the instructions provided by our team.
If your health card has expired
Please bring your expired card so our reception team can check your coverage and explain the next steps. If we cannot confirm valid OHIP coverage, an uninsured visit fee may apply. We’ll let you know the cost before your appointment.
If you need to cancel
Please provide at least 24 hours’ notice if you need to cancel your appointment. Missed appointments or cancellations with less than 24 hours’ notice may incur a $40 fee.
